Doctors prescribe it for two reasonsâ€"the first is to give opioid addicts a safe, long-term option for relapse prevention, the second to treat stubborn cases of severe chronic pain. In both these scenarios, the goal of doctors is to find an immediate solution to a serious problem.   Methadone does work really well for most people, preventing innumerable overdose deaths every year. Its primary drawback is the prolonged withdrawal syndrome it can cause. Everyone’s experience getting off methadone is different. Some people find methadone withdrawal less intense than they expected. Other people think it’s worse than withdrawing from a short-acting opioid like heroin.   If you have an opioid use disorder (addiction), then you probably have an idea of what to expect from opioid withdrawal. Feeling like you have a bad flu, vomiting, diarrhea, sweating, and insomnia are all common. With a short-acting opioid, these withdrawal symptoms can be intense, but they typically peak within a couple of days and then begin to resolve.   Because methadone is a long-acting opioid, the drug can remain active from anywhere between 8 and 59 hours, which means that withdrawal comes on more slowly and lasts longer. Acute methadone withdrawal symptoms can last up to 14 days, but many people also experience post-acute withdrawal symptoms for months after their last dose.   These lingering psychological symptoms leave many people feeling uncomfortable in their own skin, dissatisfied with their decision to get clean, and frustrated about their lack of progress.   Signs and Symptoms   Some doctors recommend continuing methadone therapy indefinitely. If you are thinking about quitting, talk with a doctor about the pros and cons. Quitting methadone can be a long and frustrating process, even for people without an opioid addiction. If you are using methadone to treat pain, you are still physically dependent on it and will experience withdrawal symptoms when you try to quit or reduce your dose. Your experience with methadone withdrawal will depend on a variety of factors, including your past experiences and expectations. Some people go into it thinking it’s going to be a nightmare, but it turns out to be milder than they anticipated. Others go into it thinking it will be easy and find that it requires a much greater commitment than they thought. Most people experience withdrawal symptoms within 2 to 4 days of their last dose. Your symptoms may start off mild and become more severe over the course of a few days. Once they reach peak severity, your symptoms will begin to resolve. After you detox, your brain is operating with depleted stores of neurotransmitters. It can take up to six months for your brain to return to normal.   During that time, you may experience symptoms such as: depressionlow energy levels  a low tolerance for stress (short fuse)  trouble sleepingthe inability to experience pleasure from anything  problems with memory or concentrationirritability or agitation   These symptoms may come and go over the course of several weeks or months. Coping and Relief The best way to cope with methadone withdrawal is to avoid skipping doses or trying to quit abruptly. Even if you are frustrated and sick of going to a clinic or pharmacy every day for a supervised dose, it is best to avoid abrupt cessation. Instead, talk to your prescribing doctor about tapering.   Tapering methadone means gradually taking smaller doses over the course of several weeks or months. Although tapering is considered the best way of detoxing from methadone, it can still be challenging. You are likely to experience withdrawal symptoms each time your dose is cut. Working closely with a doctor, you can extend your taper timeline and reduce your dose by smaller increments if necessary.   The  World Health Organization recommends reducing your dose by 5 to 10mg per week until you reach a dose of 40mg. After that, reductions should slow to 5mg per week. They acknowledge that this is only a general recommendation and that the taper schedule should be tailored to each individual’s needs.   Dose reductions can occur once a week, once every two weeks, or less often. Sometimes one dose reduction is harder than the others and uncomfortable withdrawal symptoms occur. When this happens, your doctor can pause the taper at that level and wait several weeks before reducing the dose again.     If you are experiencing withdrawal symptoms when you reduce your dose, your doctor may recommend treating your symptoms with other medications. The  American Society for Addiction Medicine  recommends doctors consider prescribing the following medications: Clonidine,  a hypertension drug regularly used off-label to treat opioid withdrawal symptoms. It is available as a pill or a transdermal patch.  Loperamide  to treat diarrhea.  Ondansetron  to treat nausea and vomiting.  Benzodiazepines  to treat short-term anxiety.  OTC pain relievers, like Tylenol or Advil, to treat aches and pains.   There is another option that you may want to talk to your doctor about:  Lucemyra (lofexidine hydrochloride). Lucemyra is a new medication; its similar to clonidine and recently approved by the  FDA to treat the symptoms of opioid withdrawal. It is the first non-opioid medication clinically proven to do so. Lucemyra works by reducing the release of norepinephrine, a neurotransmitter that researchers believe plays a role in the symptoms of opioid withdrawal.   If you are experiencing insomnia or disordered sleeping, you might also want to ask your doctor for something to help you sleep. Your doctor might give you a prescription for  trazadone or suggest an  OTC antihistamine, like Benadryl, to help you sleep. Warnings Opioid withdrawal is rarely dangerous for healthy adults, but some people need to be more cautious than others. Most notably, pregnant women.     Opioid withdrawal can cause pregnancy complications, including miscarriage and premature birth. If you are currently using methadone or other opioids, it is best to continue with methadone until the end of your pregnancy. Methadone is  proven safe and effective for use during pregnancy. It does not cause birth defects or pregnancy complications. Although it is possible that your child will be born with an opioid dependence, which means they will need to detox.     It’s important to keep in touch with both an obstetrician (OBGYN) and an addiction doctor throughout your pregnancy. Pregnancy affects the way your body metabolizes methadone, so your dose may need adjusting as your pregnancy advances.     Although the laws about opioid abuse during pregnancy do differ among the states, methadone is safe and legalâ€"you should not run into any problems when you seek treatment.   You should also talk to your doctors about your plans after giving birth. Breastfeeding is generally considered safe during methadone treatment. If you want to stop taking methadone, your doctor will talk to you about an appropriate tapering timeline. They may advise waiting until your body feels back to normal.   One last warning: Relapse is common among opioid users. After you detox from methadone, your opioid tolerance will be much lower than it used to be. If you relapse with heroin or prescription painkillers, you will be at risk of a life-threatening overdose.     Long-Term Treatment It is not unusual for people with opioid use disorders to go on and off methadone over the course of several months or years. Detoxing from methadone is a great step, but it doesn’t mean that you’ve beaten your addiction. Proper addiction treatment takes a multifaceted approach that combines medication management with psychotherapy and social support. The primary goal of long-term treatment is to prevent relapse. Relapse rates among people with opioid use disorders are very high.  Research shows that roughly 3 in 4 people who complete opioid detoxification relapse within 2 to 3 years. These numbers aren’t pretty, but don’t let them scare or discourage you. Relapse is a part of the recovery process. Knowing the odds will help you understand the value of ongoing treatment.   Remember that it is not uncommon for people to overdose and die during a relapse. Fortunately, there is a non-opioid medication available that can help reduce your risk.   Naltrexone is what’s known as an opioid antagonist (similar to the naloxone in Suboxone). Naltrexone binds to the opioid receptors and acts like a blockade, preventing other opioids from binding to those same receptors. This means that even if you relapse and use an opioid, you won’t get high (but you can overdose). Knowing that using an opioid won’t get you high should discourage you from impulsively relapsing.   Naltrexone is available with a prescription and can be taken as a daily pill or a monthly injection. Because naltrexone can bring on instantaneous withdrawal, you shouldn’t start using it until all the methadone is out of your system. This can take as long as 14 days. Research shows that a combination of talk therapy and medication management is more effective at treating opioid use disorders than medication alone. When it comes to therapy, you have a variety of options. Most methadone clinics offer some sort of counseling, so that’s a good place to start. Once you have tapered down, you might want to find something more convenient, like a therapist in private practice or at a local community health clinic. You can also look into group therapy sessions, which are often held at hospitals and other addiction treatment facilities. Finally, social support is a key aspect of relapse prevention. Many people find support at local 12-step meetings, such as Narcotics Anonymous. In the poems, â€Å"Woodchucks† by Maxine Kumin, and â€Å"The Early Purges† by Seamus Heaney, two drastic examples of killing an animal is discussed. Although both of these poems discuss the killing of animals; one is about the extermination of â€Å"pests† and the other is the killing of a family pet. Both poems tell a story of killing an animal; however, they are told from different perspectives. The following is a comparison of each of these poems and discusses how even though the narrators share a common bond; their story is very different because of their point of view, style in telling the story, and tone in describing the killings. Poets use different points of view depending on how they want their audience to react to a certain poem. In the poem â€Å"Woodchucks†, the audience sees the poem through the eyes of the killer as the events are occurring. This allows the audience to have a better understanding of the events leading up to the killings and exactly how the killings occurred. The poet says, â€Å"Now drew a bead on the littlest woodchuck’s face† (Line 17), and â€Å"Ten minutes later I dropped the mother† (Line 19). The poet describes the actions of the woodchucks as they are happening as well this is seen when she says, â€Å"Next morning they turned up again, no worse† (Line 7), and â€Å"They brought down the marigolds as a matter of course and then took over the vegetable patch nipping the broccoli shoots, beheading the carrots† (Lines10-12). Because the events are being told in present tense by the killer, the reader is able to identify her actions and have an understanding for her justification; to save her garden and vegetables. Unlike â€Å"Woodchuck†, Heaney’s poem is told from the perspective of an adult looking back on an event that occurred in his childhood. In Heaney’s poem â€Å"The Early Purges†, an adult recalls a vivid childhood memory of kittens being drowned. In the very beginning of the poem he starts out by saying, â€Å"I was six when I first saw a kitten drown† (Line 1). Although the story is being told by adult, the reader can envision a six year old witnessing this tragic event. He remembers feeling as a child that this was a cruel punishment for the kittens, and states, â€Å"Suddenly frightened, for days I sadly hung Round the yard, watching the three sogged remains turn mealy and crisp as old summer dung† (Lines 10-12). Towards the end of the poem, the narrator, now an adult, has changed his attitude toward the killing of animals. Even though there is no other reason for killing them than to get them out of the way, he acts if killing any animal is warranted. This is seen when the poet says, â€Å"And now, when shrill pups are prodded to drown I just shrug, ‘Bloody Pups’† (Lines 16-18). All the sadness and pain he once felt is gone. The people and storyline in this poem is very different from those in â€Å"Woodchucks†, where the killing of the woodchucks was necessary to stop them from eating the garden. Here, there is no reason given that can justify the killing of the kittens and the only understanding is how the narrator was changed as a person by being a witness to the drownings. Every poet has their own style in the way they compose their work. Both, Kumin and Heaney share the same approach by telling a story within their poem. Although both of their stories are about killing animals, the way in which they tell it leaves a different feeling for the animals and their killers. In â€Å"Woodchucks†, the narrator takes the readers through her thought process; first, killing the woodchucks becomes her goal; she even dreams about it, â€Å"I dream I sight along the barrel in my sleep† (Lines 27-28). This shows that she can’t stop thinking about it until she gets them. Then, she justifies her actions, â€Å"The case we had against them was airtight† (Line 4). By saying that they had a strong case against these animals, and that this was just the consequence of the woodchuck’s actions. The poet also talks about how the woodchucks are â€Å"taking the food from our mouths† (Line 13). Finally, her guilty conscience arises â€Å"If only they’d all consented to die unseen gassed underground the quiet Nazi way† (Lines 29-30). These last two lines show her guilt and how she wishes they had died when she tried gassing them so she wouldn’t feel as guilty. If they had died this way she would have not seen their faces when she killed them so it would have been less personal, but because she shot them one by one she saw each of their faces. In â€Å"The Early Purges† the setting of the farm allows for these differences to become apparent. When the narrator was younger he was exposed to these things. Although he did not agree with them, and he was frightened his exposure was a critical role in how he later justified his actions. When Dan Taggert said, â€Å"Sure isn’t it better for them now? † (Line 7). The child hearing this began to lose his innocence by witnessing the event and hearing that this action was acceptable, which leads him away from childhood and into adulthood where the poet used the farm to explain that, â€Å"on well-run farms pests must be kept down† (Line 21). This is the answer that the child that is now grown and no longer innocent gives to justify his actions in harming animals. Finally tone is a technique used in both poems, but in different ways. Even though she was able to create an â€Å"airtight† seal on both exits of the underground tunnels, the gas was ineffective because the woodchucks â€Å"had a sub-sub basement out of range† (Line 6). The second stanza describes how the problems still persists and the woodchucks are back â€Å"taking over the vegetable patch nipping broccoli shoots, and beheading the carrots† (Lines 11-12). This shows that it has now become a personal war between her and the woodchucks because they are taking what is hers. In the third stanza she describes herself as a â€Å"pacifist fallen from grace† (Line 15). In â€Å"Woodchucks† it is understandable why she had to kill the woodchucks as they were destroying her garden and eating her food. After other attempts, she was left with no choice. It is easy to feel compassion for her as even though she started acquiring a killer instinct, she felt guilty and remorse that it had come to her shooting them. In â€Å"The Early Purges†, there is more of a focus on the impacts the killing had on a child. Because there was no justification given for the killings it affected the narrator’s mind and emotional well being. He became hardened after witnessing the killing of family pets. Through each of the pieces of work, the style and tone of the poems was very different as they were from different perspectives. In â€Å"Woodchucks† the audience gets the story from the killer, herself, which provides more details in the process and in her thoughts. Whereas, in â€Å"The Early Purges†, the audience is left without answers and a rationale and instead are left wondering and pondering just like the narrator. There is a direct hotline from the devil’s mouth to my pencil. When I write I spew evil words onto the paper. Everything I touch is tainted with my weirdness. Aliens spawn in the hand that I write with. The Cold War was started by members of my cohort, a gang of evil Russian lefties. Every murderer EVER was a lefty. Ok, not really. I write with my left hand. On a slightly more serious note, spiral notebooks are my sworn enemy, inky pens are the bane of my existence (maybe I’m being a little dramatic). I write with my left hand but cut, use a mouse, play tennis, play golf, and take pictures with my right. Because I live in a righty world. Consider those little chairs in lecture halls with lift out desk tops. A prime example of lefty prejudice. They are always made for righties. When was the last time you saw a pair of lefty scissors? Lefty golf clubs are scarce; all cameras feature a button on the right side. Or at least it seems like that to me. Every couple of months an old friend discovers my left-side tendencies and exclaims, â€Å"You’re a lefty?!?! No, I would have noticed.† What am I supposed to say?: â€Å"Sorry I should have warned you. Didn’t you read the disclaimer when we became friends?† My left-sided leanings can be a shocking oddity. I realize that as I write this essay I risk sounding like a whiny teenage girl. I’ll be the first one to admit, the adversity I face as a lefty is stunningly insignificant compared to the problems other teens face every day. But this mild adversity has opened my eyes to the conventional way people think. â€Å"Left† in French is â€Å"gauche† and in Italian, â€Å"sinistra.† I detect some lefty prejudice. â€Å"Sinister† as I may be, I am usually not hurt by being a lefty. While playing sports I can switch hands on a whim. Sometimes, being a lefty has even benefitted me. Lefties have advantages in most ball sports. I can type faster: there are 3400 words that can be made solely with the left hand on a QWERTY keyboard compared to 450 with the right. And if I wanted to write in Hebrew or Arabic, it would be easier for me. Interestingly, my three passions aren’t affected by left-handedness. I chose one of the few sports, swimming, where being a lefty doesn’t change anything. I play piano, and while this may make it easier to play left-handed melodies, it’s not like guitar, a whole different instrument for lefties. My third passion is writing. I like to write things that make people smile, and it doesn’t matter what hand I write it with. Being a lefty has showed me that standardization is constricting, and differences should be embraced. My fourth passion is for chocolate. No hand inequalities there.
